The Practice now occupies a purpose built medical centre with an on-site pharmacy. The medical centre has suitable access for disabled patients and facilities include; children's play area, mother and baby room, large airy waiting room and disabled toilet facilities in addition to well equipped consulting rooms and treatment areas.
The practice is contracted to provide general medical services to patients registered with our practice and temporary residents within our practice boundary (as agreed with Calderdale Primary Care Trust). We also provide immediately necessary treatment for any person not registered with the practice who has a medical emergency within our practice boundary.

If you wish to use any of our online services on this homepage please contact the surgery to find out how to register There has been an increase in missed appointments due to patients failing inform the surgery when they are unable to attend. It has now become necessary to implement a Did Not Attend policy. If you repeatedly fail to attend appointments you may be removed from this practice list and will be asked to find an alternative doctor. If you are unable to attend your appointments for any reason please let us know as soon as possible, we can then offer the appointment to someone else We have recently installed a new telephone system. please follw the instructions when your call is answered. Please assist us by avoiding ringing at 8.30 a.m. and 12.00 p.m. if you can ring at other times. NEW PATIENTS Patients are registered with the practice, not an individual GP. Your medical card will be issued in the name of one of the doctors who will supervise your care, however you can at any time express a preference for a practitioner, for either all of your medical needs or on a case by case basis. This preference will be recorded in your medical notes. Whilst the practice will always try to comply with your requests on all occasions, it may not always be possible. The Partnership of Plane Trees Group Practice is a full equal partnership with no restriction or limitations. The partnership is contracted to provide general medical services to patients registered with our practice and temporary residents within our practice boundary (as agreed with Calderdale Primary Care Trust). Patients wishing to register with our practice should complete and submit the relevant forms available at the Practice. Our Patient Advisors will be happy to guide you through the process of registration. Since April 2004 the Primary Care Trust has advised that practices should define a 'practice boundary' for patient acceptances. For advice on our practice area please contact reception. GP AVAILABILITY GPs operate a rota system over a period of a number of weeks and are on-call on a rota basis. Whilst on-call they are available for consultation/advice. Please telephone the practice for further information on GP availability. APPOINTMENTS We are able to offer patients appointments within 2 working days or the choice of a pre-booked appointment up to 6 weeks ahead.. You can do this in person at reception, over the telephone or via the website. To make an appointment: Telephone or call when the surgery is open. Book on the day morning appointments will be available from 0830 and book on the day afternoon appointments can be booked from 1200 onwards.

You will be able to book an appointment in advance if the doctor or nurse is available or you will be offered the earliest possible alternative. Urgent cases will still be seen on the same day. You can also talk to a GP or nurse, if they are available, for telephone advice.

HOME VISITS
Contact the practice before 1100 hours. The patient advisor will need details of the problem and a doctor will phone you before the visit.
Equipment that helps diagnosis is too bulky to be carried on visits therefore, the best consultation can be carried out at the practice. A doctor can see 4 or 5 patients in surgery for the time it take to complete a single home visit.
